As a key community leader, you are invited to join the West Cook Coalition (WCC) for a small group conversation that will address community needs and assets in targeted suburbs within the Proviso and Leyden townships.

In this webinar we will be discussing some of the challenges facing your community including, but not limited to, housing stability, food insecurity, transportation, community living, disease management, and behavioral health. We need your help in building the best program for our most vulnerable residents with funding from the Illinois Department of Healthcare and Family Services (HFS) Healthcare Transformation Collaboratives. Our discussion will inform a community-wide program that we are building as part of the HFS Transformation initiative.
