The West Cook Coalition (WCC) invites community stakeholders, members, and organizations to a virtual WCC Pre-Implementation Planning Kickoff.

The WCC is a group of clinical, social service, and other community providers working together to transform healthcare delivery in the West Cook community. The WCC aims to implement a new and community-based system for delivering healthcare that encompasses both clinical care and social support.

During the virtual discussion attendees will:

1) Receive updates on WCC's vision, current activities, and future plans

2) Provide feedback on:

-- WCC's vision, activities, and plans

-- Current and pressing issues to address during a pre-implementation stage

-- Ongoing community engagement activities
